INSPECTION REPORT FOR THE ATTENUATION TANK ENSURING THE READINESS PRIOR TO THE APPLYING CHEMICALS

REPORT FOR THE READINESS CHECK PRIOR TO THE CONFINED SPACE ENTRY
The
following are observed during the inspection and were discussed with the Construction
team for preparation prior to the commencement of any activities in the
attenuation tank:
- Safe access with alternative access shall be
provided. Since the current access provided is found not safe some of the
access lengths and breadth are very narrow where safe and easy access is
not possible.
- A dedicated emergency rescue point with a
tripod and winch shall be provided. Also ensures the workers wear a full
body harness all the time while in confined spaces as per emergency
requirements.
- Ventilation and its airflow to be improved. As
it lacks ventilation inside the attenuation tanks as the exhaust system
and internal ventilation provided are found insufficient.
- An effective two-way communication system is
to be established prior to the entry of a confined space for emergency and
often monitoring of the workers' activities in confide space.
- Throughout the activity in the attenuation, limit the use and storage of chemicals and shall be controlled by establishing a daily COSHH assessment and following the SDS requirement.
- Job rotation shall be mandatorily practiced
considering the adverse weather condition, type of chemical and equipment
used, etc. This shall be enforced and monitored through the entry-exit
record by a competent person.
- All the workers entering the confide space and
the team involved in the activity must be third-party trained and daily
briefed about the risk and best practices as per their job type.
- Avoid engaging in activities on the south
and north sides of the underpass beside the live road unless sufficient
temporary traffic arrangements have been made available.
- The atmospheric check shall be conducted frequently,
and suitable fire safety arrangements shall be provided.
- Competent supervision shall be consistently
presented to monitor the activities for safety compliances.
The
concern above were discussed for the site arraignment as per confide space
safety requirements but are not limited to further safety arrangements to
improve for a better workplace.
Note*
This report will give you a basic idea for further modification and help
the safety officer lone handling a site and struggling to draft a detailed
letter to the contractor, subcontractor, department, manager, or worker in
charge. The location, the mode of operation, and the environment may vary as
per the scope of work and the geography of the workplace.
