HSE INSPECTION REPORT OF UNSAFE CONFINED SPACE ENTRY INTO THE 28-METER DEEP SHAFT AND THE MICRO TUNNELING


At (location), observed perilous activities by a subcontractor (Company name) performing the maintenance work in the 28-meter deep shaft and in the long tunnel. The activity is performing lacking the requirement of confined space entry placing the workers at extreme risk where workers could encounter a deficiency of oxygen, explosion as they carry out welding operations, Fumes and toxic gases, fall from height, etc.


Following are the major non-conformance observed during the inspection;

  1. The mechanical ventilation and the exhaust fan provided are not sufficient and they do not continue till to the bottom of the shaft and then to the tunnel. 
  2. No communication system is established since a rigger at the top sounded continuous to get the attention which longed more than 4 minutes to finally the workers to respond. Also, there is no possible and swift way to notify in the event of an emergency at both ends. 
  3. No competent whole watcher was assigned for the confined space entry where it was required considering the work to perform in the tunnel. 
  4. No rescue arrangements are available for 28 meters deep shaft and lengthy tunnel which includes a tripod. 
  5. Access is in the means of a man basket using a crane. The man basks can carry two people where seven people are in the shaft at a time. Also, both have no valid third-party certification and a permit to lift. Also, found the narrow gap to the shaft as access where the basket observed often hit the wall, and the people in the man basket pushed the wall to align the man basket to go through the narrow gap. This could lead to other occurrences of damaging the basket component considering the corroded welding joints at most parts of the man basket.
  6. Third-party certifications for confined space entry for two workers which is not certified by an approved certified trainer. 
  7. Four workers involved in the confined space do not possess the project induction ID as they clime finish the induction eight months ago, and two workers do not attend the project Safety Induction.  
  8. No alternative access is provided if the crane shut the people in the shaft will be strangled. 
  9. No signage addressing the activity and best practices around the location. 
  10. The work has no competent supervision and safety surveillance to ensure the confined space entry safety requirements are adhereed to.

All the non-compliance has been escalated to Construction management to further investigate the non-compliance and communicate with the responsible managers for appropriate action and rectification before any entry to the shaft.

The activities at the location were observed high risk to both the public and the project operatives. Hence, HSE Department hereby advises the construction team to ensure immediate rectification following confined space entry requirements and best practices as per contractor HSE system documentation and Local legislation.


Or otherwise, referring to the General Conditions of Contract sub-clause XX.X0.XX, the Health and Safety department fixes the date to achieve the final compliance as of XX.XX.XXXX and penalties may be applied as per the sub-clause XX.X0.XX, in the event of failure to comply with the instructions and we strongly advise you that to start the rectification promptly. Failure to achieve the above within the stipulated time will also be escalated to the top management, with a recommendation to hold the payment as per the specific contractual terms and conditions.

This report will give you a basic idea for further modification and help the safety officer lone handling a site and struggling to draft a detailed letter to the contractor, subcontractor, department, manager, or worker in charge. The location, the mode of operation, and the environment may vary as per the scope of work and the geography of the workplace.
