HOW DO I, A SAFETY OFFICER REPORT THE SAFETY NON-CONFORMANCES WHICH NEED URGENT ATTENTION?


Yes, there are safety matters required immediate attention though we safety officer take that up reporting to the lane management or the team head the task performed. Usually, for reporting, most of the contractor use specific for hazard notification or similar kind and is often stays in the HSE file or folder. Further, follow up of a hazard notification only happens in those company has a strong HSE culture and standards. 

Being a responsible HSE mate when you repeated HSE concerns, and activity breaching the standard safety procedure, immediate danger in life, property, public etc. HSE mate in the opinion of taking that further to management or team head on the safety non-compliances. Following are a general construction safety report send to the management through an e-mail. 

GENERAL CONSTRUCTION SAFETY REPORT 
Several activities are progressing at (site location) by (team or department name) observed numerous Health and Safety non-conformance which required imperative actions to mitigate concerns. The work location is adjacent to the residential and commercial apartments and a live traffic road. Many of the activates at the location could distract the public traffic and unauthorized entry into the construction zone by the public where both the public and the workers could endanger them into as follows;
  • Absence of fall protection around the shaft openings. The shaft has approximately (xx) meters deep for the micro tunneling and it has completed the backfilling around the shaft is reached at the top. Now the workers are performing the job around the shaft where no fall protection and no warnings available. Further observed a ladder placed to the shaft could lead to the use of reaching the top shaft and leaning over.
  • Alongside ring road observed cavities on the edge of excavations and are at risk of cave in. The excavation dug close to living traffic road without safety buffer from the edge. The risk of a cave-in is very high as the work team places heavy concrete barriers on the edge and the vibrations from the continuous movement of public traffic adjacent to it. Further observed a worker in the excavations is at risk of burying in which the edges have tension cracks and there is a sign of miner cave-in already happed. The workers and the public are at risk of being buried, fall into the excavation once the edge collapsed.
  • An excavation has no adequate edge protection. An excavation which has a depth of about 8 meters not protected its edges adequately from the risk of fall into it. The contractor has given tubular barriers (soft barriers) in some portion of the edge which has been observed moved, replaced overlapping the edge and creates a gap to potential entry into the edge and a fall into the excavation. This unsafe condition is showing the in the competency of workers, supervisors and the health and safety monitoring by the contractor. Negligence of safety requirement could lead to major occurrences.
  • Similarly, most of the edge protection provided for the excavation from a fall hazard has been fallen into the excavations. The barriers are provided on the edge without a required distance from the edge and most of the excavation is close to construction access which no way keep the equipment away from the edge. The edge protection provided could nor serve it purposed to keep away the equipment from the edge neither fall into the excavation.
  • Shaft, absence of confined space requirements whilst performing the activities in the shaft. The contractor has not ensured the permit conditions for the entry of confined space such as emergency rescue arrangements, exhaust ventilation, regular monitoring of gas test, warring and informative signage, work cycle, regular monitoring of works etc. The contractor being ignored the requirement of confined space and let the workers perform the activity in the shaft exposing all the danger of confined space hazards. This identifies the work team including the contractor health and safety the team not inspecting the confined space before the entry confirming the compliances are accurate as it one of the high-risk activity. This attitude is substantiating since 4 workers enter the confined space without a valid third party certification.
  • Absence of housekeeping at the location. The workplaces and working platform are scattered with construction materials, waste items, trailing cable, sharp edge item etc. The area becomes congested and unsafe to work as the work proceeds without competent supervision. The workers are using the power the tool, carrying materials between the scattered materials, performing lifting operations are posing a risk of the trip, fall, impalements, contact with the power tool, potential entry by the public to the worksite from the nearby residential area including their children etc. This appeared one of the major problems at the location, no effort has been taken from the work team to ensure spend little time to tidying the workplace through they could avoid incidents and attain good work attitude.
  • Poor maintenance, storage and absence of inspections of lifting equipment. The lifting equipment is on the ground which has missing of safety latches, corroded chains and it's parts, chemicals on it etc. which could substantially affect the fitness of the item and is liable to cause occurrences related to lifting or post-tensioning once it is being used at the site.
  • Use of unsafe access, a makeshift ladder observed on the site and inappropriate access to the manhole where the ladder provided inside the manhole is not secured and not provided a safe landing on the ledge. Also, missing the ladder on the other side since the workers will use a ladder to reach the top of the ledge of the manhole to gain access on the other ladder whilst that could lead them a fall from height.
The works are progressing the work at the location where a neighboring the residential and commercial acres and a live road without ensuring; barriers and perimeter fence all around the work zone, Dedicated access control and security personnel to restrict the unauthorized entry, Signage and contact details, Light arrangement for the night, Pedestrian access if applicable etc. 

All the stated Health and Safety concern has been discussed with the site HSE team for further communication and notified through the Ring HSE observer group. The contractor shall ensure timely rectification of non-conformances as stated above.
