WHAT YOU TO INSPECTION IN A MICROTUNNELLING

SAMPLE HSE/SAFETY INSPECTION REPORT OF MICROTUNNELLING (HSE/SAFETY ENFORCEMENT NOTICE)  


This inspection report of micro tunneling by the HSEmate addressing the concerns to the higher management and the team responsible for the Microtunneling process. This report shall be used as HSE enforcement notice or Safety violation report or equivalent to the project-specific HSE documentation.

(Location name) tunnel by (Company name inspected or department name) has reached the length of (xx)-meters and the length will be protracted as tunneling work progress. During works, (Your or department name) has raised numerous safety concerns consisting (document name if issued earlier if not re-arrange the phase) to the (department name) is irresponsive to make momentous corrective actions to endorse a safe place to work as the (department name) performing one of the critical activity at (project name).

(Date from and to, if no inspection is made so fare remove the paragraph), xx numbers of observation are open and overdue in (references of your site HSE hazards Tracking Matrix or relevant). Wherein, the severity rate of six (6) observations are categorized under ‘’critical’’ and sixteen (16) observation are high risk. (the risk rating may change as per your documentation and project requirement)

The infringement of safety requirements and significant safety concerns have observed during the inspections requires a considerable rectification as follows (but not limited to): 
  • (Company name inspected or department name) has no safety procedure available whilst entering and performing the maintenance work in xx-meter depth shaft and xxx-meter length tunnel. Thus, the (company name inspected or department name) workers entering the tunnel without a rescue procedure and emergency equipment, no awareness of the risk involved in the task they performed, frequent communication is not established, etc. and as follows;
  • Periodic inspection is not carried out by a competent person to monitor any adverse development in the shaft and the tunnel would adversely affect the stability.
  • Unsafe and disorganized electrical connection and services. Also, no emergency stop witch available at a strategic location that can stop the entire electrical system in the event of an emergency.
  • Absence of fire emergency or fire suppression equipment and its arrangements in the shaft and to the tunnel.
  • No continuous dewatering occurred at shaft thus, water accumulated in the shaft which could substantially affect the shaft integrity also, limit the space of movement, slip, trip, ill-health etc.
  • No frequent gas monitoring. And no smoke detection and rate of temperature rise available in the shaft and tunnel.
  • Lack of ventilation; The mechanical ventilation provided to the shaft is not reasonable and inadequate to cover the entire shaft and tunnel. Many occasions the ventilation system found unserviceable.
  • Pressurized hose connections at the shaft not whip checked and deteriorated pressure pipes/hoses.
  • Unauthorized storage and use of chemicals in the shaft which is toxic and flammable.
  • Lack of Personnel protective equipment; (company name inspected or department name) workers enter the tunnel without a berating aperture. (company name inspected or department name) has one self-contained breathing apparatus without a valid calibration certificate which is inadequate the number of workers performs the task in the tunnel.
  • Excessive leakage of slurry and water from the separation plant that resulted in the water accumulation on the edge that dripping into the shaft. Surplus weighs on the edge would adversely affect the shaft edges and wall.
  • Absence of rescue equipment from the tunnel and from the shaft to a safe zone where emergency services can reach.
  • The food is supplied in plastic bags and unauthorized containers and stored together with construction material and chemicals in an unhygienic wooden box.
  • Use of unauthorized diesel tank posing fire hazards and environmental issues as it has an undue leak of fuel.
  • Smoking in a non-designated area; (company name inspected or department name) staff found smoking inside the operator control room of TBM.
  • The general arrangement of the materials and equipment (housekeeping) found unsatisfactory.
  • The makeshift ladder shall replace with a stair as observed workers are continuously using the ladder during the tunneling process.
  • Absence of competent supervision; Designated lifting supervisor unviable. A (company name inspected or department name) a supervisor who performs as lifting supervisor and have multiple responsibilities at deferent locations. 
(company name or department name) shall reinstate their own Health Safety and Environment System documentation, HSE Plan Section: (reference) Leadership and Commitment and Section: (reference) Subcontractor Management considering the type of job they performed which require immense attention. And shall ensure rectification in line with the (local authority or contract reference), international best practices and own HSE management document. The same shall apply to all the similar and future activities at (project name).

This report will give you a basic idea for further modification and help the safety officer lone handling a site and fraught to draft a detailed letter to the contractor, subcontractor, department, manager or work in charge. The location, the mode of operation and the environment may vary as per the scope of work and the geography of the workplace.
