A SPECIFIC INSPECTION REPORT OF SCAFFOLDING IN MICROTUNNELLING

UNSAFE SCAFFOLDING IN THE MICROTUNNELLING SHAFT AND NO SIGNIFICANT RECTIFICATION UNTIL AFTER THE ENFORCEMENT NOTICE (DOCUMENT REFERENCE NUMBER IF ANY) ISSUED.


The below report is specific for scaffold erected inside the micro tunnelling shaft. The scope of the scaffold to perform the maintenance works on the 28-meter-deep shaft wall and access to the shaft and to the levels of working platforms. 

The team performed the inspection in the shaft has undergone to the formal confined space and work at height training. The detailed inspection report and findings are below is specifically for scaffolds. 

Details of report
Collective inspection carried out today (date here) with (company or department name) Scaffolding engineer Mr (name here) and HSE engineer Mr (name here) on all the scaffolds erected in and around at the micro tunnelling (MT) shaft. The inspection is conducted to validate the rectifications and action by the contractor on the recent enforcement notice (document reference number) and consecutive email communications to the (contractor, department or manager name) on (date) (Unsafe Scaffolding in the project and bypassing the standard requirements). The main area of focus was on all the shaft at (specify the locations) where the most scaffolding has been erected for access to and from the shaft and to perform the work. All other the shaft has view decks which were in the decent condition during the time of inspection. 

(your department name) raises the apprehension of dispersed scaffolding team which has no consistency in managing, inspecting and handing over certification with a tag under the supervision of contractor project scaffold engineer Mr (name). Throughout the inspection at MT shafts, it was learned that the MT shaft has a separate team to install the scaffold under MT management subsequently by the (your department name) advice. Which is then not issued any handing over certificate as required by the enforcement notice before commencing the works. 

At (specify the locations) scaffolding is tagged red as it found noncomplying the scaffolding requirements after the (document reference number if any or continue without any remanence and date) issued on (date here) for rectification such as: 
  • Absence of guard rails and toe boards around the platform in the Shaft. 
  • Unsecured scaffold planks. 
  • Gaps in the working platforms. 
  • Absence of supports and bracings. 
  • The build-up of dust all around the scaffold and platform could lead to slip and fall. 
  • Unsafe access ladder as it angles is not maintained safe and extension of landing is not as per 1:4 ration etc. 
The scaffold platform erected in the (specify the locations) for the access and to perform the work is found dangerously wobbling which has the depth of xx-meter is an example that the scaffold is not erect firm and as per specification. The scaffold is tagged green allowing the workers to gain access and exit and to perform the work placing them into an extreme risk of scaffold collapsed into the shaft. Also, this scaffold was used with a green tag without a handover certificate. All these unsafe conditions indicate that there is no parodic inspection is functioning from the contractor by a competent person confirming safe to work. 

(your department name) strongly advise the contractor, working in the Micro Tunnelling is high risk as the depth is varying xx to xx meters. Hence, the number of inspection to be increased, only the work shall be performed after a handover certification by the competent site scaffolding engineer. Since then all the work on the scaffold in and around the microtunnelling shaft shall be halted until it is rectified and authorised by (company or department name) Scaffold engineer. The rectification shall follow the safe system of scaffolding, work at height requirement and best practices as per contractor HSE system documentation and local legislation.

Note* 
This report will give you a basic idea for further modification and help the safety officer lone handling a site and struggling to draft a detailed letter to the contractor, subcontractor, department, manager or work in charge. The location, the mode of operation and the environment may vary as per the scope of work and the geography of the workplace.
