This is to write the final warning letter to (Company Name) Crane mechanical Supervisor Mr (name of violator), failing to make sure a safe workplace and placing his operative into an extreme risk into a fatal incident.
Maintaining a safe and healthy work environment is critically important and it is reasonable for a supervisory consultant to expect that the contractor will cooperate with the project health and safety management to promote a safe workplace and safety of each employee. It is everyone responsibility and duty of care for the health and safety of themselves and the people in the workplace by complying the project HSE system documentation and the Local legislation.

The following matters concerning the above-mentioned employee overlooked to comply with the requirement as he was directly responsible for the activity performed; 
(Insert location of violation), a (Company Name of violator) Mechanic was observed positioned awkwardly on the tier mounted crane and leaning himself towards between the boom and hydraulic jack while the boom was lowering. The Mechanic stated that he has been advised by the supervisor (Name of violator) to check the mechanical condition as the crane operator experienced an unusual sound from the mechanical parts. While doing so, it was observed the mechanic was at high risk of being trapped between the mechanical part and could severely be injured and could lead to a fatal incident. The supervisor of violator instructs the operator to do the job dangerously putting the worker life at extreme risk and the supervisor becomes a spectator rather doing it in a safe way. When notified the unsafe way of doing the job the supervisor resisted with aggressive behaviours claiming it is safe to do so. Similar kind of incompetent supervision could invite fatal incident at the site. 

Supervision consultant concluding that the above (Company Name of violation) Staff bypassing the Health and Safety requirement and there is no reasonable excuse for the deliberate misconduct placing the worker in extreme danger. If there is any further unsatisfactory conduct of similar or any action placing the workers into risk, the same shall be escalated to the client and severe disciplinary the action shall be initiated.
